Job summary

Trilogy Health Services, LLC is seeking a Physical Therapist to deliver a full range of PT services at our Mount Washington facility. You will assess, plan, and apply therapeutic interventions to improve patient function, while educating families and maintaining thorough documentation.

Collaborate with an interdisciplinary team in a supportive environment. Travel is minimal; licensure in the state of Kentucky is required, with benefits starting after a short period.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• Current state PT licensure in good standing; CPR preferred.
• 0-1 years of PT experience in a healthcare setting.
• Ability to work in an interdisciplinary team and follow regulations.

Responsibilities

• Conduct thorough assessments to evaluate patients' physical conditions and rehab needs.
• Develop and implement personalized treatment plans with exercises and modalities.
• Educate patients and families on exercise techniques, posture and body mechanics.
• Maintain accurate documentation of progress and outcomes per policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
