EmpowerMe Wellness is seeking licensed therapists (PT, PTA, OT, COTA, or SLP) to join EmpowerMe 360, delivering therapy and wellness services to older adults across Castle Rock communities. You’ll help residents stay independent, safe, and engaged, guiding assessments and personalized care plans with a collaborative team.

Formación

• Must hold a current professional license in your therapy discipline (PT, PTA, OT, COTA, or SLP).
• Passion for empowering seniors to live active, independent lives.
• Strong clinical skills and collaborative teamwork.

Responsabilidades

• Provide therapy and wellness services for older adults.
• Conduct assessments and create personalized care plans.
• Deliver evidence-based interventions with a collaborative team.
• Support care coordination with wellness check-ins and telehealth.
• Maintain professional standards aligned with company initiatives.
• Contribute to program development and quality improvement.
