Qualifications

• Current California Physical Therapist (PT) license in good standing.
• Experience working with older adults is preferred, but motivated PTs with other clinical backgrounds are encouraged to apply.

• Ability to work independently while collaborating effectively with a multidisciplinary team.

Responsibilities

• Provide high-quality, individualized physical therapy evaluations and treatments for older adults.
• Work with residents across Independent Living, Assisted Living, and Memory Care.
• Develop and implement evidence-based treatment plans based on each resident’s functional goals.
• Address mobility, balance, strength, endurance, gait, fall prevention, transfers, and functional independence.
• Collaborate with caregivers, nursing staff, physicians, and other rehabilitation professionals.
• Educate residents and caregivers on safety, mobility, and home exercise programs.
• Complete required documentation accurately and in a timely manner.
• Monitor progress and modify treatment plans as clinically appropriate.
• Participate as a valued member of the senior living community and interdisciplinary care team.
