Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

$20,000 Sign-On Bonus - Full Time

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) to perform home visits for our Ocean County Senior Living team. This role involves working with clients on a per visit basis within senior living communities throughout Ocean County. We welcome new graduates to apply!

As a home care Physical Therapist, you will be an integral part of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care at home.

Responsibilities of a PT - Physical Therapist:

• Make home visits to clients in designated geographic areas.
• Perform diagnostic tests and measurements, including mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
• Implement protective interventions to prevent wound formation and promote wound healing using evidence-based dressings and topical agents.
• Develop and execute individualized care plans, including manual therapeutic exercises, gait training, and balance interventions.
• Continuously assess and update the care plan, collaborating with nursing staff as part of a multidisciplinary team.
• Educate clients and their families on rehabilitative care and activities to promote health, safety, and independence.
• Document observations, interventions, and evaluations accurately using a state-of-the-art tablet.

Qualifications:

• Current NJ Physical Therapist (PT) license.
• Graduation from an accredited program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computer systems; prior EMR experience preferred.
