Job summary

UnitedHealth Group is seeking a Senior Living Physical Therapist for a full-time role serving adult to geriatric patients. Responsibilities include comprehensive assessments, direct treatments, supervision of PT assistants, home exercise program development, and patient/family education in an outpatient setting.

Requirements include current PT licensure in the state, reliable transportation, and CPR (preferred).

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, or access to dependable transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates patient to determine rehab potential and sets an individualized plan of care and goals.
• Sets initial short-term and long-term goals.
• Uses and maintains equipment safely; logs inspections and maintenance.
• Maintains outpatient clinic equipment logs.
• Follows medical regimens and adheres to precautions (THR, TKR, cardiac, wound, etc.).
• Uses safe treatment techniques: transfers, positioning, edema management, cryotherapy, ultrasound, electrical stimulation, etc.
• Completes discharge summaries on final outpatient visits.
• Ability to function in diverse home settings and with diverse clients.
