Home Health (Visits) Occupational Therapist

BAYADA Home Health Care is currently seeking an experienced Occupational Therapist for a part-time opportunity providing exceptional rehabilitative therapy to geriatric patients in Senior Living Communities (Assisted Living and Independent Living). As an Occupational Therapist, you will deliver therapeutic services to geriatric patients throughout Ocean County.

Home Health Occupational Therapists are integral members of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing, enabling clients to receive necessary medical care to remain at home.

Occupational Therapist, OT Responsibilities:

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
• Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans.
• Evaluate the home environment for hazards or barriers to independent living.
• Identify equipment needs and environmental adaptations to improve outcomes.
• Continuously assess and revise the occupational therapy care plan, collaborating with nursing in the multidisciplinary care plan as appropriate.
• Educate and instruct patients, family members, or representatives in rehabilitative care and activities to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.

Occupational Therapist, OT Qualifications:

• Current New Jersey Occupational Therapist license.
• Prior home care experience preferred but not required.
• At least one year of recent work experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program as indicated by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
