Qualifications

• Must hold a valid nurse practitioner or physician assistant license in the state of operations.
• Master's degree in nursing or physician assistant studies required.
• Strong leadership, interpersonal, and problem-solving abilities are preferred.

Responsibilities

• Provide direct patient care and comprehensive primary care in senior living facilities.
• Conduct comprehensive assessments and develop treatment plans.
• Perform routine rounds and monitor residents' health conditions.
• Coordinate care with facility staff, families, and other providers.
• Educate residents and families on wellness and disease management.
• Maintain accurate EMR documentation and support quality improvement.
