StoryPoint Northfield is seeking a Nurse Supervisor/LPN to lead and train a team of caregivers in our assisted living/memory care setting. You’ll evaluate residents, report changes to physicians, and coordinate care using EHR and EMAR systems. Strong communication with residents, families, and staff is essential.

Qualifications

• Licensed Practical Nurse (LPN) in Kentucky
• Prior supervisory experience required
• Assisted Living/Memory Care experience preferred

Responsibilities

• Lead, manage, and train your team of caregivers.
• Evaluate residents and report changes in condition to physicians.
• Utilize Electronic Health Records (EHR) and Electronic Medication Administration Records (EMAR).
• Communicate effectively to team, residents and families.
• Build strong relationships with residents, families, and staff.
