Linden Ponds by Erickson Senior Living in Hingham, Massachusetts, is seeking an LPN/LVN to provide compassionate clinical care to residents in our neighborhood, in line with nursing standards and Erickson Senior Living policies.

You will develop resident relationships, assess and document clinical status, update care plans, administer medications on schedule, supervise care associates, and participate in resident outcomes meetings, with a focus on safety, dignity and well-being.

Qualifikationen

• Licensed by State as a Licensed Practical Nurse.
• CPR certification required.
• A passion to care for seniors and be a strong resident advocate.
• Minimum of 1 year of experience in assisted living, home health, or long-term care is preferred.
• Experience with electronic medical records is preferred.

Aufgaben

• Develop relationships with residents and families using a consistent caregiver approach.
• Assessing, observing, and evaluating residents' clinical condition and documenting findings in the EMR.
• Updating or modifying care plans as needed to reflect changes in care and service needs.
• Supervising and leading care associates.
• Administering medications timely, accurately, and according to orders.
• Participating in meetings related to resident outcomes and care.
• Responding to residents, family, or responsible parties in a timely and professional manner.
• Participating in the review of clinical indicator reviews (falls, wounds, nutrition, etc.).
• Other duties as assigned by RN clinical leader.
