Job summary

Charter Senior Living in Sylvania, OH is seeking an LPN to support residents in our assisted living and memory care communities. The role includes resident assessments, care planning support, medication administration, and accurate clinical documentation.

This position offers day and night shifts (7a–7p and 7p–7a) plus PRN, with starting pay between $27–$31 per hour. We value compassionate, team‑oriented care and provide growth opportunities and a supportive work environment.

Qualifications

• Active LPN license in good standing.
• Experience in assisted living or memory care preferred.
• Strong medication management and clinical assessment.
• Excellent communication and teamwork abilities.

• Ability to work in a fast-paced environment while maintaining attention to detail.
• Strong organizational and documentation skills.

Responsibilities

• Provide nursing care and clinical support to residents.
• Administer medications and treatments per orders.
• Assist with resident assessments and care planning.
• Monitor resident conditions and report changes.
• Maintain accurate clinical documentation and records.
• Collaborate with families, physicians, and care teams.
• Support resident dignity, comfort, and quality of life.
• Assist with infection control and quality assurance.
• Mentor caregiving staff as needed.
• Foster relationships with residents and team members.
