Reata Glen in California is seeking a Resident Health Services Director to oversee nursing evaluation, care coordination, and education for residents across wellness programs. You will partner with medical staff and families to ensure high-quality services bridging residential and skilled care.

The role requires California LVN licensure, CPR/First Aid/AED, and at least five years leading teams in long-term care or similar settings; RCFE preferred.

Qualifications

• Current California LVN license in good standing.
• RCFE preferred.
• CPR, First Aid, and AED required.
• Five years’ experience working with seniors in long-term care or similar health care setting is preferable.
• Five years organizational or management experience; supervising others is preferable.
• Must have a passion for working with the elderly, possess strong clinical skills, and have strong customer service, leadership, and interpersonal skills.

Responsabilités

• Provide nursing evaluation of residents and coordinate care with medical personnel and family members, and follow-up after emergencies.
• Coordinate resident returns including transportation, medications, and a light meal; update plan of care.
• Provide individual and group educational opportunities for residents and staff in disease prevention and wellness.
• Facilitate with other leaders in the continuum of care to identify and recommend additional services or move within the continuum.
• Anticipate and coordinate with residents and families when significant changes require additional support or transfer to higher level of care.
• Assist residents and families by offering support in dealing emotionally with aging and illnesses.
• Provide day-to-day oversight of medication technicians and LVN staff to ensure standards are met; manage clinic services including pre-admission health screening.
• Support for management of chronic conditions, coordination of clinical visits by other professionals, and assistance with medications and ADLs.
• Responsible for interviewing, selection, training, supervision and performance management of all residential health services staff.
• Responsible for ensuring all staff training is conducted and documentation maintained.
• Other duties as assigned by supervisor.
