Job summary

EmpowerMe Wellness seeks a Care Coordinator to partner with Area Directors to optimize care in senior living communities. You will manage schedules, support intake, and coordinate with therapists to ensure comprehensive resident care and family communication.

The role requires strong organizational skills, attention to detail, and the ability to multitask in a fast-paced environment while maintaining confidentiality and high-quality service.

Qualifications

• Certification in healthcare, business administration, or related field.
• Two years of relevant experience in a senior living setting preferred.
• Experience in administrative and operational roles, including managing schedules, handling correspondence, and maintaining records.
• Solid understanding of wellness offerings and therapy services for seniors, and knowledge of Assisted Living/Memory Care regulations.
• Excellent communication and interpersonal skills, with a patient-focused approach.
• Ability to stay calm under pressure, manage interruptions, and multi-task effectively.

Responsibilities

• Build caseload by engaging with residents and coordinating care needs.
• Oversee intake processes and gather documentation with consent from residents/POA.
• Provide care coordination and wellness activities in communities.
• Schedule evaluations and coordinate with clinical staff and therapists.
• Support onboarding and orientation of new team members; act as interim Clinic Director when needed.
• Review operational reports and assist in closing gaps in care coverage.
