Job summary

M2 Healthcare in New York, NY seeks an Admissions Coordinator to manage patient admissions from hospitals, senior living facilities and community partners. You will screen referrals, coordinate with clinical teams, and ensure paperwork, tours, and onboarding are seamless.

Responsibilities include building relationships with discharge planners, families, and providers; promoting the center in the community; and ensuring regulatory staffing standards are met.

Qualifications

• Minimum of 2 years' experience in skilled nursing or hospital setting.
• Excellent written and oral communication skills.
• Must be a team player, well organized, self-motivated, and flexible.
• Experience with admission coordination, marketing plans, and public relations preferred.

Responsibilities

• Establishes and maintains professional relationships with community members, hospital leadership, discharge coordinators, caseworkers, physicians, senior living communities, senior centers, and admission sources to generate admissions and referrals.
• Acts as the initial contact for discharge planners, families, and other responsible parties to respond to referrals; may require after-hours responses.
• Facilitates daily communication with discharge planners and community representatives to assess needs and manage the admissions process.
• Screens and reviews potential admissions; collaborates with clinical team to identify care goals and ensure resources and services are available.
• Works with Administrator, DNS, and regional leadership in admissions decision making; exercises ethical and sound judgment.
• Coordinates staffing with Administrator and DNS to maintain well-being and meet regulations.
• Manages the admission process for each new patient, including screening, paperwork, and preparing patient rooms.
• Completes admission paperwork with patient or agent and communicates information to the admissions team.
• Develops programs to promote the center to hospitals and agencies in line with marketing plans.
• Provides tours to prospective patients and agents, highlighting services and differentiators.
