Centerville Place Assisted Living in Centerville, OH, is seeking an Activities Assistant to plan, organize, implement, and evaluate recreational, social, and spiritual programs for residents. You will work with the Activities Director to create a monthly calendar, assist with transportation, document attendance, and support resident well-being in a compassionate environment.

Qualifications

• High school diploma or equivalent required.
• Must be able to successfully pass background check.

Responsibilities

• Plan, organize, implement, and evaluate recreational, social, intellectual, emotional and spiritual programs.
• Create monthly activity calendar and post in a prominent location visible to residents.
• Interview residents or family members to obtain activity preferences.
• Coordinate and assist residents in getting to activity locations within the facility.
• Assist in conducting scheduled activities to promote resident well-being.
• Document activity attendance and chart progress notes with resident responses.
• Observe mood and involvement; report changes to Activities Director.
• Involve residents and families in planning activities when possible.
• Assist with in-room activities for one-on-one visits.
• Coordinate offsite field trips and transportation as needed.
• Coordinate with other departments to ensure daily activities run smoothly.
