Sanford Health in New Hope, MN, is seeking an activity-based care support role. The position involves administering the monthly activity calendar, engaging residents, and assisting with basic nursing tasks and patient transport.

It requires CNA certification or current enrollment in a healthcare program, with BLS/CPR training appropriate to the location. Candidates should have a high school diploma, experience in geriatrics or social services, and strong communication and leadership skills.

Qualifications

• High school graduate or equivalent preferred.
• Experience in social services or creative activities preferred.
• Experience in geriatrics preferred.
• Current Certified Nursing Assistant (CNA) certification or enrollment in a healthcare related program preferred. Basic Life Support (BLS) required based on location. Must obtain CPR certification within 6 months of hire.

Responsabilités

• Administers the monthly activity calendar and plans engaging activities.
• Regularly elicits patients'/residents' input into activity planning and keeps activities organized and upbeat.
• Assists with simple nursing procedures and transporting patients/residents.
• Maintains MDS/EPIC documentation and demonstrates strong written and verbal communication.
• Demonstrates leadership to develop and motivate staff and plans resource usage.
