Job summary

Catholic Charities Brooklyn and Queens, Inc. is seeking a Case Manager III to deliver comprehensive services to homebound elderly clients in the community, helping them live safely at home with a personalized care plan.

The role includes in-home assessments, developing and implementing care plans with clients and families, crisis intervention, emergency coverage for Home Delivered Meal clients, intake activities, and timely documentation to support funding requirements and client outcomes.

Qualifications

• Bachelor’s degree in Human services or related field.
• Knowledge of entitlement for older adults preferred.
• Proficient in computer software applications, Excel and Microsoft Office.
• Bilingual preferred.
• Able to travel to client homes, meetings and training.
• Ability to lift up to 10 lbs.

Responsibilities

• Provide a comprehensive in-home assessment of functional and cognitive capacities and eligibility for benefits.
• Develop and implement a care plan with the senior and family to address unmet needs.
• Provide crisis intervention for medical emergencies, utility shut-offs, bedbug infestations, and disasters.
• Provide emergency coverage for Home Delivered Meal clients when needed.
• Perform intake activities and provide information and referrals.
• Complete a comprehensive DFTA assessment form for each client on time.
• Maintain up-to-date case notes, waiting lists, and monthly statistics/reports.

Job description

For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.

Through the Benson Ridge and the Southwest Queens Senior Case Management Programs, case management and the homecare services are offered to the homebound elderly. The services include offering care management plans, assessments for the homecare, connections with the Home Delivered Meals and Friendly Visiting. The goal of the Case Management Program is to provide vital support services to keep frail seniors living safely in their homes and communities.

STATEMENT OF THE JOB:

The Case Manager III provides comprehensive services to homebound elderly clients in the community through support services to maintain the client at home with a safe care plan.

• Provide a comprehensive in-home assessment and reassessment of the older person’s current functional and cognitive capacities and limitations, unmet needs, and eligibility requirements for benefits and entitlements.
• Develop and implement a care plan with the senior and his/her family when needed to address the client’s unmet needs and assist them in continuing to live safely in their own homes.
• Provide crisis intervention services when needed for situations such as medical emergencies, utility shut-offs, bedbug infestations, and disaster response.
• Provide emergency coverage for Home Delivered Meal clients when the client fails to answer the door when needed.
• Perform intake activities, which include assessing eligibility, preliminary evaluation of the presenting problem, initiating services as needed, and providing information and referral services.
• Complete a comprehensive DFTA assessment form for each client on time and according to funding source regulations.
• Maintain up-to-date case notes, waiting lists for clients on caseload, and the timely submission of monthly statistics and reports as required by the agency and the funding source.

QUALIFICATIONS:

• Bachelor’s Degree in Human related services from an accredited college or university.
• Knowledge of entitlement for older adults preferred.
• Proficient in computer software applications, Excel, and Microsoft Office.
• Bilingual preferred.
• Able to travel to client’s homes, meetings, conferences, and other locations for training.
• Ability to lift to 10lbs.
