Senior Hospice Patient Care Manager Leader - RN - $10,000 Sign on Bonus

Heartland Hospice, part of the Gentiva family, is seeking a Senior Hospice Patient Care Manager RN to join our team. We provide physical, spiritual, and emotional support to patients and their families, ensuring quality care and professional growth for our staff.

Responsibilities include:

1. Facilitating and participating in the Interdisciplinary Group (IDG) meetings, including presiding over meetings and coordinating patient reviews.
2. Managing clinical and administrative functions such as documentation, referrals, authorizations, and insurance verifications.
3. Supervising clinical staff, assigning visits, and ensuring timely completion of patient visits.

Qualifications:

• Graduate of an accredited nursing school; bachelor’s degree preferred.
• Current RN licensure in the state of residence.
• Minimum three years of direct patient care experience, with at least two years in hospice or home care.
• Knowledge of hospice principles, regulations, and industry standards.
• Ability to oversee program management and ensure compliance and quality.
