PTMatch is seeking a dedicated Home Health Physical Therapist to provide in-home evaluations and therapy across Philadelphia-area counties, including Philadelphia, Montgomery, Bucks, and Delaware. Full-time, salaried, and treated as a leadership team member with opportunities for pay steps based on weekly visits.

The ideal candidate will have PA PT license, active CPR/BLS, and a compassionate, independent work style, delivering patient-centered care with strong documentation and collaboration

Qualificações

• Current Physical Therapist license in PA is required.
• Graduate from an accredited Physical Therapy program.
• CPR/BLS certification preferred or required per policy.
• Home health experience is preferred but not required.
• Ability to work independently in a home setting with team communication.

Responsabilidades

• Perform in-home PT evaluations, assessments, reassessments and treatments per physician orders.
• Develop and implement individualized plans to improve mobility, balance and independence.
• Educate patients and caregivers on home exercise, safety and fall prevention.
• Monitor progress, update goals, and communicate changes to the care team.
• Complete accurate clinical documentation in line with standards and regulations.
• Collaborate with nurses, therapists, physicians, families, and other team members.
• Supervise PT Assistants where applicable per state policy.
