Immediate Home Health and Hospice seeks a dedicated Physical Therapist to join our home-based care team in Philadelphia, PA. You will partner with patients to restore motion and maximize function in their own homes, with a focus on evidence-based care and individualized plans.

• Proven work experience as a physical therapist.
• Graduate degree or greater in physical therapy.
• Valid license to practice physical therapy.
