VA White River Junction Healthcare System seeks an RN for Home Base Primary Care in Brattleboro, VT. The role is full-time (40 hours/week) with responsibilities spanning direct patient care, education, and coordination across settings to support veteran-centered outcomes.

Qualifying candidates must have approved nursing education, state licensure, and the ability to work in home-based primary care, with potential for bridge programs and licensure progression as needed.

Qualifications

• English proficiency required for direct patient care.
• Graduate nursing degree from an approved program with state licensure.
• Option for bridge program leading to licensure as a nurse.
• Licensed nurse with eligibility for permanent appointment after licensure.
• Foreign nursing education requires degree equivalency validation.

Responsibilities

• Provide and supervise direct patient care using the nursing process.
• Coordinate patient care across settings with emphasis on education and self-management.
• Administer medications and perform clinical procedures per policy.
• Lead quality improvement and patient satisfaction initiatives.
• Deliver evidence-based care to Home Base Primary Care patients in veterans' homes.
