Job summary

Lancaster County seeks a Senior Support Coordinator to manage a caseload of infants and toddlers born with opioid exposure, guiding families through Early Intervention services and coordinating care with a network of providers. You will outreach to birth mothers, conduct family assessments, and monitor progress while documenting in statewide databases.

Ideal candidates have strong casework skills, the ability to navigate multiple systems, and a commitment to trauma-informed approaches.

Qualifications

  • Two years of experience as a caseworker.
  • Bachelor's degree in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
  • Or any equivalent combination including 12 college credits in related social sciences.

Responsibilities

  • Carrying a specialized caseload of infants and toddlers with opioid dependence/prenatal exposure.
  • Outreach to birth mothers in NICU or after discharge to discuss Early Intervention services.
  • Developing, implementing, and reviewing IFSPs including social work services and transition planning.
  • Documenting activities in the Pelican database and CARES/ELSE systems; ensuring accurate data entry.
