• Plan and direct a comprehensive social services program for residents and families.
• Supervise nursing home social workers and coordinate with the interdisciplinary care plan team.
• Maintain written records of social service consultations and services provided.
• Coordinate discharge planning and community resources as part of residents’ care plans.
• Act as liaison between residents, families and outside agencies to protect residents’ rights.
• Perform administrative duties and ensure facility compliance with policies and safety codes.
