Job summary

CVS Health is seeking a Lead Director of Digital Product for Chronic Condition Management. You will shape technology strategy and end-to-end execution for chronic condition programs, overseeing Epic architectures and data integrations to support safe, high-quality outpatient care.

You will lead a multidisciplinary team and partner with executives to drive value-based care initiatives while optimizing patient and provider experiences.

Qualifications

• Education: Bachelor's degree required; graduate degree and/or clinical background preferred (RN, MD, DO, NP, PA).
• Experience: 10+ years of progressive leadership spanning healthcare operations, clinical technology product management, and Epic.
• Track record partnering with primary care networks, retail clinics, and value-based care organizations to improve medical cost trends.

Responsibilities

• Partner with healthcare delivery leadership to design end-to-end care models and reduce administrative burden.
• Drive cross-functional technology teams to deliver care model strategies and technology products.
• Establish shared outcomes and ensure technology investments improve clinical quality, patient experience, and cost.
• Lead teams to translate clinical ideas into Epic/technology architectures and robust roadmaps.
• Ensure integration of external data sources with Epic workflows for real-time insights.
