3+ years of acute care hospital Case Management experience.
2+ years of Case Management leadership experience.
Strong data analysis and action-planning skills.
Excellent communication and leadership; strong physician and post-acute relationships.

Responsibilities

Lead and manage daily hospital Case Management operations.
Oversee patient care coordination, discharge planning, and continuity of care.
Ensure optimal utilization of clinical resources and achievement of department outcomes.
Provide executive leadership, SME guidance, team building and change management.
Direct staffing, onboarding, competencies, and resource allocation to meet goals.
Ensure timely, high-quality care coordination, discharge planning, and documentation.
Analyze data and drive sustainable performance improvements.
Ensure compliance with policies, SOPs and regulatory requirements.
Collaborate with CMO, CNO, and senior leaders across divisions.
