Job summary

Mount Olivet Rolling Acres seeks a Lead Direct Support Professional Medical Coordinator to assist clients with daily living activities, healthcare coordination, and program implementation in a residential setting. The role emphasizes safety, supervision, documentation, and collaboration with the IDT and nursing staff.

The position involves driving to appointments, overseeing medications, meal planning, and maintaining person-centered care in a diverse, mission-driven environment.

Qualifications

• Must pass Department of Human Services background check and maintain qualified status.
• Must not be excluded from government health programs by OIG.
• Must have valid driver’s license and vehicle insurance as required by state law.
• Successful completion of Medication Administration class with 80% or higher.
• Able to meet physical job demands as required.

Responsibilities

• Assist clients with daily cares, medical needs, ADLs and mental health supports.
• Plan and implement recreation, leisure and community activities.
• Drive to appointments and accompany clients using a company vehicle.
• Maintain MAR and ensure up-to-date medication lists; monitor safety.
• Document data accurately and complete daily logs; maintain confidentiality.
