Job summary

Wheeler Clinic is seeking a Senior Counselor-Community Support for Families in the Hartford/Manchester, CT area. The role provides direct services, case management and coordination of care for an assigned caseload, interfacing with DCF, schools and other supports to secure needed resources.

The position offers a $23.08 per hour wage, a $2,000 sign-on bonus, and comprehensive benefits. Valid driver's license and strong communication skills are required for community-based work.

Qualifications

• BA/BS degree required in psychology, social work, counseling or related field, other experience considered on a year-for-year basis.
• Experience with clients with mental health and/or co-occurring substance use disorders preferred.
• Strong time management and written/verbal communication skills required.

Responsibilities

• Provide direct services, case management and coordination of care for assigned clients/families.
• Facilitate group sessions as needed.
• Maintain documentation for Medicaid/licensing requirements and reporting.
• Engage with DCF, school, probation and other supports to access services.
• Transport clients to appointments when needed and participate in on-call coverage.
