Job summary

Neighborhood Healthcare seeks an Occupational Therapist to design and implement therapy plans for seniors to maintain independence within their communities. The role includes supervising support staff, performing mobility assessments, and advancing patient care plans in a collaborative, multidisciplinary setting.

Schedule is Monday–Friday, 8:00am–4:30pm, with a $5,000 signing bonus. Strong communication, documentation, and adherence to safety and regulatory standards are essential for success.

Qualifications

• Valid CA Occupational Therapist license required and in good standing.
• One year of experience with frail or elderly population.
• Current Basic Life Support (BLS) certification required on hire; CPR and First Aid must be maintained.

Responsibilities

• Develops and implements occupational therapy programs for patients.
• Performs assessments to determine mobility and physical abilities.
• Creates individualized care plans to maximize independence.
• Monitors support staff and ensures plans are followed.
• Guides use of adaptive devices and self-care activities.
• Maintains treatment records and progress notes.
• Conducts home and site visits as needed.
• Ensures compliance with safety, HIPAA, and regulatory requirements.
