Job summary

NYC Health + Hospitals/Metropolitan in New York's East Harlem area seeks a qualified community liaison/counselor for the Ambulatory Care Administration. The role centers on counseling, outreach to residents, schools and local organizations, and supporting city-driven public service programs.

This full-time position requires 3 years of related experience or equivalent education, with preference for a high school diploma or equivalent.

Qualifications

• Three (3) years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or
• Education and/or experience equivalent to 1 above. Completion of an approved six (6) month training program of combined classroom training and on-the-job experience in community liaison work may be substituted for six (6) months of experience. Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of thirty (30) semester credits for each year of the experience described above. However, all persons must have at least one (1) year of the full-time experience described above.
• Note: Preference will be given to applicants with at least a high school diploma or its educational equivalent approved by a State’s Department of Education or a recognized accrediting organization.

Responsibilities

• Performs liaison duties between City agencies, residents, and community organizations.
• Directs staff or coordinates field office operations.
• Consults with residents, landlords, community organizations, and schools.
• Explains programs and policies to community groups and relays information.
• Participates in planning and implementing community programs; reports on outcomes.
• Assists in mediating problems between the community and schools, hospitals, and City agencies.
• Works with an interdisciplinary team to provide supportive therapy and referrals.
