Essex Partnership University NHS Foundation Trust is seeking a Band 6 Senior Community Mental Health Practitioner to join the Dementia Service across the North East Essex area. The role focuses on assessment, treatment, and support for people with dementia in their homes, care homes, and community settings.

You will work within a multidisciplinary team, aiming to keep people in their communities and reduce avoidable hospital admissions, while contributing to service development and quality

Qualifications

• Band 6 Senior Community Mental Health Practitioner within the Dementia Service.
