Job summary

A leading home care provider in San Jose is looking for a Caregiver II - Certified Nursing Assistant (CNA). This role focuses on providing compassionate care and support to patients in home and clinic settings. Qualifications

• Minimum of one year of experience with elderly or frail populations.
• Experience with seniors with dementia.
• Bilingual proficiency preferred.

Responsibilities

• Provide personal care and support per individualized care plans.
• Observe and report changes in patient conditions.
• Support onboarding and training of Caregiver staff.
