Job summary

Qualifications

• Bachelor's Degree Graduate of an accredited school of Pharmacy Required
• PharmD Doctorate of Pharmacy (PharmD) Preferred
• Licensed as a Pharmacist by the State Board of Pharmacy in the state based on facility location Upon Hire Required
• 4-6 years Minimum of 3 years experience in comparable health system pharmacy Required and ASHP residency or 3 years clinical/operational experience Required

Responsibilities

• Coordinate care across continuum of care with health care professionals.
• Serve as resource person to support pharmacy personnel and healthcare professionals.
• Design and coordinate clinical pharmacy services.
• Train pharmacy staff.
• Monitor and manage drug therapy in designated patient population.
• Develop care plans and guidelines including drugs and delivery modalities.
