Job summary

East Tennessee Children's Hospital seeks an experienced clinical pharmacist to practice at an advanced level and provide comprehensive pharmaceutical care through independent clinical decision making and medication therapy management.

The role partners with physicians, nurses, and other clinicians to optimize therapy, lead medication safety and quality improvement initiatives, and mentor pharmacy staff and students within the department.

Qualifications

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy with health system training.
• Active Tennessee pharmacist license required.
• Minimum three (3) years of relevant pharmacy practice experience; or completion of an ASHP accredited PGY1 residency.

Responsibilities

• Performs all duties and responsibilities of Pharmacist I.
• Provides comprehensive medication therapy management and clinical consultative services.
• Collaborates with physicians and other healthcare professionals to optimize therapy and outcomes.
• Leads or participates in medication safety, quality improvement, formulary management, and performance initiatives.
• Collects, analyzes, and reports clinical data to support quality improvement and safety initiatives.
• Develops and delivers educational programs and training for pharmacy staff and learners.
• Serves as a preceptor for students and residents; supports professional development.
• Assists with policy development and implementation of evidence-based practices.
