Job summary

Community Clinic of Maui, Inc. is seeking a Lead Community Health Worker to contribute to comprehensive patient care. This role involves coordinating patient navigation, monitoring care flow, and leading a team of Community Health Workers (CHWs).

The ideal candidate must possess a High School Diploma, fluency in English, and preferred experience in community-based and multicultural settings. Comprehensive benefits are included along with a focus on chronic disease management and patient support.

Qualifications

• Experience in a community-based setting; one to two years preferred.
• Experience working in a multi-cultural setting.
• Experience in a leadership role.

Responsibilities

• Ensure patients navigate efficiently to care and completed services.
• Coordinate care management and self-monitoring with patients and families.
• Oversee daily workflow and task prioritization for Community Health Workers (CHWs).
• Recruit, select, train, and manage CHWs.

Skills

Written and oral fluency in English

Good communication skills

Ability to communicate in a trauma-informed manner

Experience in a multicultural setting

Education

High School Diploma or its equivalent
