Qualifications

• Nursing Diploma or Associate's degree required.
• Bachelor's degree preferred for advancement.

Responsibilities

• Facilitates identification and resolution of gaps in care across hospitalization to ambulatory care.
• Leads development and implementation of complex care plans with interprofessional teams.
• Supports evidence-based practice and disseminates new knowledge.
• Evaluates patient and family experience and contributes to quality improvement.
• Maintains safety-sensitive performance as part of the role.
