Job summary

Qualifications

• Active RN or LVN license in the applicable state in good standing.
• Minimum two years of clinical experience as an RN/LVN.
• Experience in Case Management, Utilization Management, acute/post-acute care, or related setting.
• Strong clinical assessment, critical thinking, prioritization and problem‑solving skills.
• Ability to manage multiple responsibilities in a fast-paced environment with timelines.

Responsibilities

• Comply with Case Management, Utilization Management, and regulatory requirements.
• Independently manage a complex caseload across acute inpatient and post-acute settings.
• Review medical records to assess medical necessity and level of care using criteria and guidelines.
• Coordinate clinical reviews and communicate findings to leadership as appropriate.
• Collaborate to establish safe discharge plans early in admission.
• Assess, plan, implement, coordinate, monitor, and evaluate care management interventions.
• Provide education and outreach to Members and families to promote engagement in transition-of-care plans.
• Conduct onsite visits as assigned to support member engagement and progression to appropriate care.
