You will promote inter-disciplinary communication with patients, families, physicians, and the health‑care management team to deliver quality, cost‑effective outcomes.

Required: Bachelor’s Degree in Nursing and TN RN license; 2 years clinical experience and 3–5 years in case management; ACMA accreditation preferred.

Qualifications

• Bachelor’s Degree in Nursing is required.
• Master’s Degree in Nursing is preferred.
• Case Management 3–5 years of experience required.
• Clinical healthcare (acute or hospital) 2 years required.
• Registered Nurse license in TN required.
• Accredited Case Manager (ACMA) certification preferred.

Responsibilities

• Collaborate with the Case Management team to support quality patient care.
• Coordinate discharge planning, authorizations, and post‑discharge activities.
• Identify patients eligible for case management using screening and assessment.
• Educate patients and families; communicate with stakeholders.
• Document and monitor assigned cases; manage resources and length of stay.
