Job summary and purpose

The Senior Case Manager II is the subject matter expert for the case management team. The Senior Case Manager II will mentor the team and, as directed by the Program Coordinator, arrange, coordinate, monitor, and ensure all delivered services meet clients’ needs. This includes assisting case managers in assessing and reassessing client/household needs, educating on program and community resource opportunities, developing service plans, scheduling appointments, and providing necessary follow-up to ensure service plans progress on schedule. The position requires a combination of administrative/office duties and field/outreach work.

Responsibilities
  • Conducts comprehensive screenings and assessments with clients to collect information on functional, environmental, financial, employment, housing, educational, and health topics to develop a case plan. Develops support systems to meet client needs and coordinates move-in activities, rental negotiations, and service referrals as required. May make home and field visits.
  • Monitors case managers to determine quality and effectiveness of services; reviews cases with the Program Coordinator and Case Management Team; audits case files for completeness and accuracy; conducts crisis intervention as necessary.
  • Assists case managers by identifying and coordinating a variety of services for veterans and families to maintain/obtain self-sufficiency and family stabilization; makes referrals to VA/community providers and provides ongoing case conferencing, counseling, and advocacy.
  • Conducts comprehensive file reviews and assessments of data to ensure documentation for program services eligibility; maintains security of client records.
  • Develops, performs, and assists with workshops pertaining to program services; participates in outreach events; performs other duties as assigned.
Essential duties
  • On-site service delivery required; incumbents must report to work on-site each day.
  • Lead a team of Case Managers on their active caseload, reviewing plans of action to meet client needs, set goals, and obtain resources.
  • Responsible for intake, assessments, service planning, and service delivery.
  • Manage hard and soft file management and HMIS processes to ensure client documentation meets requirements.
  • Collaborate with other service providers to link participants to community resources and supportive services.
  • Conduct weekly case conferencing and monthly case file audits to ensure records are complete and accurate.
  • Develop and assist with workshops pertaining to veteran households’ needs; participate in outreach events; other duties as required and may be subject to change.
Mandated reporter

All employees are considered Mandated Reporters and must report any instance of physical or sexual abuse to proper authorities.

Qualifications

Requirements:

  • Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications
  • Current TB screening and Immunization Records (within 5 days of hire)
  • Copy of Current Professional License (if applicable)
  • Current BLS Certification (within 15 days of hire)
  • Valid RealDriver's License or US Passport
  • Completion of all required trainings (within 30 days of hire)

Education:

  • Bachelor’s degree from an accredited institution in the social sciences or family therapy
  • Completion of VOALA Case Management training within 6 months of hire

Experience:

  • Two years of demonstrated case management experience with high needs/high intensity of service populations

Preferred qualifications:

  • Master’s Degree (Social Work or related)
  • Experience in supervision or lead role preferred
  • HMIS navigation
