Job summary

ScionHealth in Denver seeks a Care Coordination professional to manage patient care across the continuum. You will collaborate with the Interdisciplinary Care Transitions team to ensure efficient utilization of resources and compliant care delivery.

The role requires knowledge of payor practices, Medicare processes, and accreditation standards, along with strong communication and critical thinking skills. Travel is minimal and English proficiency is essential.

Qualifications

• Knowledge of government and non-government payor practices, regulations, standards and reimbursement.
• Knowledge of Medicare benefits and insurance processes and contracts.
• Knowledge of accreditation standards and compliance requirements.
• Ability to demonstrate critical thinking, prioritization and time management.
• Basic computer skills with MS Office.

Responsibilities

• Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians.
• Monitor all areas of patients’ stay for effective care coordination.
• Refer high risk patients for additional support.
• Serve as a patient advocate to maximize informed decisions.
• Collaborate with clinical staff on plan of care and discharge planning.
