A leading community service organization in New York is seeking a Senior Case Manager to provide effective case management for formerly homeless clients with mental illness. This role requires a Bachelor's degree, relevant experience, and strong communication skills to guide clients towards better life choices. The position offers a full-time schedule with a salary range of $53,000 to $60,000 annually.
Organization Overview:The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We are two of New York City’s leading human services organizations meeting the needs of children, youth and families, homeless individuals, and older adults through-out Manhattan and New York City. We work every day for a fair and just society where all people have the opportunity to make choices that lead to better lives for themselves and their families. The Isaacs Center focuses primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational.
Program Description: Goddard Riverside’s Corner House provides affordable, permanent supportive housing for 34 previously homeless adults many of whom have mental illness, substance use, and other co-occurring disorders. On-site services include case management, entitlement assistance, medical and psychiatric care, substance use counseling, group activities, recreation, and 24 hour/7 day per week front desk security.
Position Summary: To deliver effective case management services using a strengths-based, client-centered approach to formerly homeless adults diagnosed with a severe and persistent mental illness in order to maintain permanent housing. The Senior Case Manager will provide administrative support while carrying a caseload. The Senior Case Manager will supervise, guide, oversee employee performance, and take control of implementing improvements on a daily basis. As a Senior Case Manager, you must demonstrate Integrity, flexibility, problem-solving, direct moving projects along faster and improve efficiency. Senior Case Manager must demonstrate strong decision-making skills and clear communication.
Schedule: Monday-Friday 9:00am-5:00pm
Salary Range: $53,000-$60,000

Roles, Responsibilities and Essential Duties
1) Client service
Assist clients in receiving and maintaining entitlements
Assist clients in understanding and complying with building rules to maintain residency
Maintain case records with documentation of services provided and outcomes
Responsible for case notes and service plans using an electronic database
Coordinate meetings with clients and building management
Coordinate and track medical appointments between tenants’ and various medical providers
2) Administrative Duties
3) Participate as Corner House team member
4) Other duties as required in accordance with agency polices and funding source requirements
