Job summary

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Must be able to travel locally within hiring county (60–70%).
• Proficiency with case management documentation tools and real-time data entry.

Responsibilities

• Manage a caseload of members with complex health and social needs.
• Spend ~65% of time in the field visiting homes, shelters, and community settings.
• Conduct in-person visits, assessments, and follow-ups.
• Coordinate services across healthcare, behavioral health, and community resources.
• Document interactions in real-time using internal systems.
• Develop and manage individualized care plans and track outcomes.
