Kindred Hospital Louisville at Jewish Hospital seeks an experienced professional to coordinate and facilitate patient care across the continuum. You will collaborate with the ICT team, monitor stays, and ensure optimal resource use while aligning with discharge planning and external review guidelines.

You will support care transitions, engage with families, and advocate for patient needs, contributing to cost-effective, high-quality care delivery in a long-term acute setting.

Formación

• RN license and state licensure required
• Postsecondary nursing or social work education required
• Bachelor’s degree in nursing or social work preferred

Responsabilidades

• Care coordination with Interdisciplinary Team and Physicians
• Monitor patient stay for effective care coordination
• Refer high risk patients for additional support
• Advocate for patients and families to support informed decisions
• Participate in interdisciplinary rounds and discuss treatment goals
• Plan and evaluate care across the continuum of care
