Job summary

You will mentor staff, lead workflows, collaborate with leadership and interdisciplinary teams, and drive quality improvement, policy development, and performance measurement in case management.

Qualifications

• Five (5) years of previous case management experience.
• Current Maryland unrestricted licensure as an RN or PT.
• Membership in a healthcare or case management organization.

Responsibilities

• Serve as expert resource for case management practice and regulatory compliance.
• Lead development and evaluation of workflows and care management processes.
• Collaborate with leadership to maintain policies and operational guidelines.
• Monitor practices and integrate evidence-based standards to improve outcomes.
• Provide mentorship and professional leadership to staff.
• Lead quality improvement, accreditation readiness, and population health initiatives.
• Foster collaboration with providers, payers, and community organizations.
• Evaluate outcomes to identify opportunities for improvement.
• Represent the department in professional activities.
• Perform all other duties as assigned.
