You will supervise junior carers, document daily activities and coordinate with other health and care professionals to ensure high-quality, person-centred care. Flexible full-time or part-time hours available to match service user needs.

Responsibilities

• Act as Senior Key worker, formulate care plans and update as needs change.
• Supervise junior staff, provide guidance, PPE, and carry out reviews and assessments.
• Report issues to management and maintain records of daily activities and hours.
• Ensure service delivery aligns with policies and clarify weekly hours with service users.
• Support service users to achieve outcomes through personal and instrumental daily living activities.
• Engage with professionals and signpost to services to support needs.
• Assist community integration and promote well-being in collaboration with stakeholders.
