Job summary

One Senior Care seeks a Master Social Worker to be at the heart of the participant experience, building relationships quickly, listening deeply, and helping seniors and families navigate challenges with confidence and care.

You will guide participants through care plans, connect them with resources, and advocate across departments. This role suits someone who excels in conversation, collaboration, and direct impact through people-centered work.

Qualifications

• MSW from an accredited School of Social Work.
• At least 1 year of experience with frail/elderly populations.
• Valid state driver’s license and reliable transportation.

Responsibilities

• Build trusting relationships with participants and families.
• Participate in initial assessments, care planning, and ongoing re-evaluations.
• Provide individual and family counseling with a supportive, person-centered approach.
• Lead group discussions and activities to strengthen emotional well-being.
• Document case management records timely and accurately.
• Coordinate with community resources and advocate with outside agencies.
