Job summary

Helia Healthcare, LLC is seeking a Social Services Director to oversee psychosocial needs assessment, discharge planning, and family coordination for residents. You will lead a dedicated team, ensure compliant documentation, and collaborate with physicians and staff to deliver holistic care.

The role requires a social work degree, geriatric experience, and a valid state license, with Medicaid/Medicare familiarity and strong leadership and communication skills.

Qualifications

• Bachelor’s degree in social work or a related human-services major.
• At least one year of social work experience with supervisory duties and direct work with geriatric residents.
• Current Missouri or Illinois Social Work license (location dependent).
• Experience with Medicaid/Medicare and regulatory knowledge.
• Leadership and communication skills strong enough to guide a team and collaborate with healthcare professionals.

Responsibilities

• Assess residents’ psychosocial needs, risk factors, life stories, quirks, and family dynamics.
• Design social service interventions to improve quality of life.
• Complete required documentation in a timely, organized manner.
• Develop discharge plans that ensure safety for residents and staff.
• Lead the social services team with charisma and structure.
• Coordinate Family Council and manage grievances with professionalism.
• Collaborate with families, physicians, staff and referral sources to align care.
• Track quality indicators to support continuous improvement.
