Job summary

Executive Home Care of Omaha is seeking a motivated Referral Marketing Representative to grow our referral network in the Omaha metro. This community-based, relationship-focused role connects with SNFs, hospitals, ALFs, and other senior care providers to facilitate transitions to in-home care.

You will visit facilities, present services, educate partners about programs, and collaborate with leadership on outreach strategy.

Qualifications

• 5+ years in social work, discharge planning, case management, healthcare, or senior care.
• Experience with hospitals, SNFs, rehabilitation facilities, ALFs, memory care, or similar settings preferred.
• Understanding of healthcare referral and care transition processes.
• Ability to travel throughout the Omaha metro area.
• Strong communication and relationship-building skills.
• Comfortable with face-to-face networking and professional outreach.

Responsibilities

• Build and maintain professional relationships with discharge planners, social workers, case managers, and healthcare professionals.
• Develop referral partnerships with SNFs, rehabilitation facilities, ALFs, memory care communities, hospitals, and other senior care organizations.
• Conduct regular in-person facility visits, introductions, and follow-ups.
• Present Executive Home Care's services and value proposition to referral partners.
• Educate healthcare professionals and facility partners about programs like Sensi Co-Pilot and GUIDE.
• Serve as a reliable point of contact for referral sources and respond to questions or requests; deliver marketing materials and maintain communication.
• Identify new referral and business development opportunities throughout the Omaha metro.
• Track outreach activities, referrals, and relationship development.
• Represent Executive Home Care at community events, lunch-and-learns, networking events, and in-service presentations.
• Collaborate with EHC leadership on outreach strategy and territory development.
• Help grow and strengthen the referral network and community presence.
