Job summary

Oasis Therapy Services is seeking a Physical Therapist Assistant (PTA) to provide one-on-one rehabilitation services at Charleston House Senior Living in Beaver Dam and Eden Vista Madison in Madison, Wisconsin.

Responsibilities include working under a licensed PT, following treatment plans, educating residents and caregivers, and documenting progress in Net Health. The role requires Wisconsin PTA licensure, CPR, and travel between sites with flexible scheduling.

Qualifications

• Active and current Wisconsin Physical Therapist Assistant license.
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.
• Reliable transportation and ability to travel between Beaver Dam and Madison.

Responsibilities

• Provide treatments under the direction and supervision of a licensed Physical Therapist.
• Follow treatment plans addressing mobility, strength, balance, endurance, and transfers.
• Monitor residents' responses and report changes to the supervising Physical Therapist.
• Educate residents and caregivers on exercises, safe mobility, and fall prevention.
• Complete accurate Net Health documentation and collaborate with the care team.
