Job summary

Humboldt Senior Resource Center is seeking a Center CarePartner to provide personal care to Redwood Coast PACE participants in the Day Center and during transit to appointments. The role supports bathing, grooming, feeding, vital signs monitoring, and coordination of meals per each participant’s Plan of Care.

The ideal candidate has at least 1 year experience with frail/elderly populations and CPR/First Aid certification within 6 months.

Qualifications

• Must have at least 1 year of experience working with a frail or elderly population.
• CPR and First Aid certification required within 6 months of hire.
• Excellent interpersonal and teamwork skills.

Responsibilities

• Assists participants with bathing, grooming, toileting, and feeding.
• Accompanies participants to medical appointments as needed.
• Monitors vital signs and maintains supplies.
• Coordinates meals and participates in Plan of Care.
• Completes documentation accurately and promptly.
