Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist with current license to practice
• Evaluate residents in skilled nursing and post-acute facilities
• Develop individualized treatment plans to address ADLs, mobility, cognition and safety
• Document in EMR and maintain accurate patient records
• Collaborate with an interdisciplinary team to coordinate care
• Educate families and caregivers about treatment plans and progress
