Senior Care Options (SCO)

The SCO nurse case manager acts as a liaison between the patient, their family and healthcare personnel to ensure efficient, prompt health care services and to minimize fragmentation and duplication of services. Care plans for SCO patients both the medical necessitates as well as psychosocial needs of each patient and are developed in collaboration with other SCO team Members to ensure the most cost effective plan is implemented. Nurses work closely with patient and in their families in creating a care plan in order to adjudicate the patient’s wishes for health care with the goal of maintaining independence and safety in the community. Nurse case managers are expected to control the health center resources and home services through periodic reviews of the care plan, maintain continuity of care between home, health center and hospital, provide nursing centered office visits and home visits. In addition, excellence in this position includes global skills and capabilities beyond baseline competence. These include demonstrating and comprehensive understanding of the department’s immediate needs and long term growth goal. Schedule: Monday - Friday, 8:00 AM - 5:00 PM
Starting at $44.00 per hour, and up to $64.00 based on years of relevant experience.
